New to the clinic? Here’s some important information.

I want to make your first experience with my services as seamless as possible. So whether you’ve already booked your first appointment with me or are preparing to do so, below are some important pieces of information you’ll want to have so that we can have the best first experience possible.

Download and fill out forms

When you book your first appointment you can fill out an intake and consent form online at the time of booking.

By filling out both the Intake Form and COVID-19 Informed Consent Form in advance it will give you more time for your first treatment.

If you have problems filling out the forms online you can download them by clicking on the links below. (The forms will open in a new window.) Simply download the forms, fill them out, and bring them with you to your appointment.

Directions to my clinic

My clinic is located in the Evergreen Business Centre, found in the plaza at 2950 Douglas Street. The best entrance to use is to enter off of the Burnside Road entrance (ideally closest to the Winks Convenience Store). There is an elevator in the business centre lobby that you can use if you are not comfortable taking the stairs.

What to do when you arrive

When you arrive for your appointment you will find my office just off the main lobby at the top of the stairs, if the door is open please come in. If the door is closed have a seat in the lobby and I will get you when I am ready to start your appointment. (Please note that the receptionist helps with the greeting of clients but does not take payments; I’ll take care of that at the end of your session.)
